Family Therapy for a Daughter with Anorexia Nervosa

Abstract
Objectives: This article describes the use of family therapy with a girl diagnosed with anorexia nervosa. The author explored the factors that affected the client¡¯s symptoms, the family therapist¡¯s intervention method, and the factors that helped reduce the symptoms.

Method: The family therapist treated the client and her family with an approach that combined psychodynamic family therapy, MRI¡¯s communication model, and Murray Bowen¡¯s family systems theory. In researching this case study, the author employed coding and Miles and Huberman¡¯s (1994) network and time-ordered effect matrix.

Results: Factors influencing the client¡¯s anorexia included her distorted cognitive structure; the dysfunctional communication method among the nuclear family members; the parents¡¯ experiences from their families of origin; their transmitted expression method; transference; the client¡¯s confrontational method, and the absence of a communication partner for the client. To reduce the symptoms of the client¡¯s anorexia nervosa, the therapist arranged for the client to recognize her problem; he relabeled, he explained differences between the couple¡¯s families of origin; and he asked the family about attempted solutions to resolve the problem. Factors affecting the reduction of the client¡¯s anorexia included changes in the mother-daughter relationship; the father¡¯s recognition of his problem and his efforts to change; the mother¡¯s recognition of the couple¡¯s dysfunctional communication method and her efforts to change; the mother¡¯s recognition of a communication method that had been transmitted from her parents-in-law and her parents; changes in the father and the daughter¡¯s communication method; and individual changes in the client.

Conclusions: A family-based therapeutic approach to treating adolescent anorexia nervosa can be effective.
