Family Therapy for a Daughter with Anorexia Nervosa

Abstract

|
|
Objectives: This article describes the use of family therapy with a girl diagnosed with anorexia nervosa. The author explored the factors that affected the client’s symptoms, the family therapist’s intervention method, and the factors that helped reduce the symptoms.
Method: The family therapist treated the client and her family with an approach that combined psychodynamic family therapy, MRI’s communication model, and Murray Bowen’s family systems theory. In researching this case study, the author employed coding and Miles and Huberman’s (1994) network and time-ordered effect matrix.
Results: Factors influencing the client’s anorexia included her distorted cognitive structure; the dysfunctional communication method among the nuclear family members; the parents’ experiences from their families of origin; their transmitted expression method; transference; the client’s confrontational method, and the absence of a communication partner for the client. To reduce the symptoms of the client’s anorexia nervosa, the therapist arranged for the client to recognize her problem; he relabeled, he explained differences between the couple’s families of origin; and he asked the family about attempted solutions to resolve the problem. Factors affecting the reduction of the client’s anorexia included changes in the mother-daughter relationship; the father’s recognition of his problem and his efforts to change; the mother’s recognition of the couple’s dysfunctional communication method and her efforts to change; the mother’s recognition of a communication method that had been transmitted from her parents-in-law and her parents; changes in the father and the daughter’s communication method; and individual changes in the client.
Conclusions: A family-based therapeutic approach to treating adolescent anorexia nervosa can be effective.
